New Patient Registeration

  • Begin your Registration. Tou'll need the following information on hand to proceed.

  • You must be at least 18 years of age or older to enter Telehealth Treatment at Marula Rehab.

  • Address and Phone number of all previous treatment programs

  • You will need to name of your primary care doctor and his or her phone number.

  • If you are on probation or parole, you will need to provide your supervising offier's contact information.

  • Insurance information if you have health insurance.

  • Your Driver License or State Issued ID and Social Security Number.

Schedule

Your Intake

Upon completion of the patient registration process, you will be required to schedule your initial intake appointment. This appointment serves as the formal starting point of your clinical engagement with Marula. During the intake session, you will meet with a Marula Specialist who will conduct a structured clinical assessment, review your presenting concerns, gather relevant medical and psychosocial history, and evaluate your eligibility for available treatment services. The information obtained during this assessment will be used to formulate an individualized treatment plan that aligns with your identified needs, clinical priorities, and the scope of care offered within the program.
